A new study exploiting the natural experiment of post-war British sugar rationing suggests that limiting sugar intake in the first 1,000 days of life may reduce the risk of several cancers in adulthood.

Researchers analyzed health data from 64,761 people born in the UK between 1951 and 1956, drawn from the UK Biobank, and compared those who spent their early development under sugar rationing with those born after the restrictions ended. Sugar rationing, introduced in January 1940 during World War II, ended abruptly in September 1953, after which average sugar consumption quickly doubled. This sharp cut-off allowed researchers to compare individuals with different levels of early-life sugar exposure.

The study, published in PNAS, found that longer exposure to sugar rationing during the first 1,000 days of life—from conception to about age two—was associated with a lower incidence of five types of cancer: liver and intrahepatic bile duct cancer, prostate cancer, lung cancer, rectal cancer, and breast cancer.

Beyond cancer incidence, the researchers reported that sugar-rationed babies had longer telomeres, equivalent to approximately 2.2 years less biological aging. They also had lower levels of Granzyme B, a marker of chronic immune activation, suggesting reduced inflammation in adulthood.

Mechanisms: Biological and Behavioral

The study's authors, health economists Chen Zhu from China Agricultural University and Weilong Zhang from the University of Cambridge, propose two pathways that may explain the long-term associations.

The first is biological: early nutrition may shape metabolism, organ development, and immune system function in ways that persist for decades. The second is behavioral: individuals exposed to sugar rationing early in life tended to consume less sugar, eat smaller quantities of food, and maintain healthier, more diverse diets approximately five decades later, suggesting that early taste preferences and eating habits may endure.

Weilong Zhang, quoted in The Conversation, said there appear to be two mechanisms at work: one biological—early nutrition shaping metabolism, organs, and the immune system—and one behavioral—a taste for less sweetness formed early that persists into adulthood.

The findings suggest that nutritional conditions during early development may have consequences for cancer risk many years later, adding to a growing body of evidence linking early-life nutrition to long-term health outcomes.

Context and Limitations

The study builds on previous research using the wartime rationing period as a natural experiment, which has already linked early sugar restriction to lower risks of chronic diseases, Alzheimer's, and better cardiovascular health.

The current analysis exploits the unique historical circumstances to examine cancer risk specifically, offering a rare opportunity to study causal effects in humans where randomized trials would be unethical or impractical.

Excess sugar consumption has long been implicated in cancer through mechanisms including insulin resistance, chronic inflammation, and metabolic reprogramming. The new study provides further evidence that the timing of sugar exposure, not just total intake, may be critical.

However, as with any observational study using historical data, the findings carry inherent limitations. The analysis relies on historical records and population-level changes in diet, and individual sugar consumption was not measured directly. The researchers note that the associations are observational, and while the natural experiment design strengthens causal inference, other factors changing at the same time cannot be fully ruled out.

Nevertheless, the study offers a compelling glimpse into how early-life nutrition can shape health across the lifespan, and it raises important questions about the long-term consequences of sugar consumption during critical developmental windows.
